Instructional Support K-8 Social Worker
Job Description
Provide a range of services for students who demonstrate difficulties with learning, academic achievement, social/emotional functioning, and behavior. Serve as a psycho-educational service provider involved in proactive, preventative activities, as well as responding to identified concerns. Provide services based on a combination of problem-solving, consultative and direct service models.
Responsibilities and Duties:
Though it is impossible to list all roles the School Social Worker will fill, specific job requirements include:
● Work with the school’s Mental Health Team to develop individual, group or system level interventions that improve identified problems or concerns, including social/emotional/behavioral/ mental health needs as they relate to educational progress
● Implement and support teachers in school wide Social Emotional Learning (SEL) using the School’s SEL Curriculum
● Collaborate and partner with the School Administration and other staff as needed to support student mental health and social emotional needs (both through whole classroom support, through individual student support, and through school community support)
● Conduct, interpret, and implement student assessments which includes record review, interview, observation, and standardized assessments
● Facilitate functional assessment of behavior (FBA) and design/implement and progress monitor behavior intervention plans
● Participate in interdisciplinary meetings to determine the eligibility, appropriate planning, and re-evaluation for students who require special education services/programs or 504 services
● Input information into EzEdMed for reimbursement purposes
● Serve on school and/or district-wide committees
● Serve as a liaison among families, community, and collaborate with various teams to assure positive outcomes for all learners
● Serve on the crisis management team (prevention and intervention) and participate in managing crises if/when they occur
● Create, organize and update case history records, and prepare reports, according to deadlines
● Provide information, when appropriate, to outside support services to help students with economic, emotional, or physical issues
● Conduct Suicide Risk Assessments, Threat Assessments, and their associated re-entry meetings
● Support Suspension re-entry meetings as necessary
